Book engine model provider

With one provider enabled, that provider drafts two independent candidates for every outline, chapter, introduction, and acknowledgement, and judges between them itself. With both enabled, each provider drafts one candidate and they're judged against each other. This also governs book title suggestions and the in-book chat.
